Welcome to Family Counseling Associates! We are glad you chose to partner with us. This page contains important instructions that you must complete prior to your first appointment.
Step 1) Pay Your Deposit.
We require a deposit payment to confirm a first appointment. The deposit payment for an initial medication evaluation is $150.00, and the deposit payment for an initial psychotherapy visit is $75.00. Your deposit payment will be applied to the fee for your first appointment. You will have the option to store a payment method for future charges when you complete your New Patient Registration form in the portal.
On the Transaction Receipt page, click "CONTINUE" to be directed to our client portal page.
Step 2) Log in to the Portal.
We use a portal to communicate and exchange important information with you. The portal is a secure platform for exchanging healthcare related information. You will receive an email inviting you to the portal from the address firstname.lastname@example.org. Please check your junk folder if you do not see it in your inbox. This email will contain the URL link for the portal, as well as your auto-generated username (the email you provided to us) and password, and you will be prompted to create your own password when logging in for the first time.
The portal is optimized for Google Chrome on computers and tablets, and smartphone users may experience complications.
First, locate the Patient Information tab on the left-hand side menu, and enter your Patient Information*. Be sure to include all required fields, including name, date of birth, address, and contact information. When finished, click “Update.”
*If you would like us to file claims to your health insurance plan, please email us at email@example.com with photos of your insurance card, front and back.
Next, locate the Forms to be Completed tab on the left-hand side menu and select New Patient Registration. Please enter as much of the information in this form as possible, as this will allow your provider to make the most of your time in your first appointment. Complete the form with all required fields and signatures, and choose "Finalize."
The Patient Information and New Patient Registration information must be completed prior to your first appointment.

Step 3) Watch for Your Reminder Email. You will receive a courtesy appointment reminder email approx. 48 hours before your scheduled appointment. If you need to re-schedule or cancel your appointment, please notify us more than 24 hours in advance to avoid a cancellation fee.
